To get your teen ready to be apart of Ignite Youth Ministry, please fill out these forms for your High School or 7th/8th grade teen!

Please fill out and sign Form *A and B either online and email to jguilday@sacredheartwf.org OR print, fill out and bring to the first youth event your teen plans to attend. 

*IF your teen has filled out a Form A with Religious Education, let Jennifer Guilday know and we can retrieve it and it will not need to be filled out twice! However, all other forms listed will need to be filled out specifically for Youth Ministry. 
Form B will need to be filled out once yearly for the weekly Youth Ministry events, and then will be filled out additionally for any events outside of these weekly events.
